Tandis que Mois de la sensibilisation \u00e0 l'autisme<\/a> est reconnue en octobre, nous voulions partager cette histoire d'impact \u00e0 l'avance en raison de l'appel urgent \u00e0 l'action pour soutenir la population \u00e2g\u00e9e de l'Ontario. <\/strong><\/p>

Isabel et Arlene (gestionnaire de cas en sant\u00e9 mentale g\u00e9riatrique, Cota) ont commenc\u00e9 \u00e0 travailler ensemble apr\u00e8s qu'Isabel ait \u00e9t\u00e9 orient\u00e9e vers Cota par Reconnect. Arlene a travaill\u00e9 avec les anciens prestataires de soins de sant\u00e9 de la communaut\u00e9 et a identifi\u00e9 les besoins essentiels pour aider Isabel \u00e0 atteindre ses objectifs. Apr\u00e8s un an de collaboration, Arlene constate que<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t

Le COVID-19 a perturb\u00e9 la prestation des services de sant\u00e9 communautaire et de nombreuses personnes n'ont pas \u00e9t\u00e9 en mesure d'acc\u00e9der aux services en face \u00e0 face dont elles avaient besoin. Cette situation est particuli\u00e8rement critique pour les personnes \u00e2g\u00e9es de l'Ontario et les personnes souffrant de troubles mentaux et cognitifs.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t
